Brief Guidelines on How to Complete the Relevant Forms

AIBB Trading Account Application Form
1. Fill in your full given name, National Identity Card Number (NRIC) (if you are a Malaysian citizen) or International Passport Number (if you are a Malaysian Permanent Resident or a non-Malaysian citizen) and permanent address in the Account Opening form, exactly as per your NRIC or your International Passport. 2. Fill out complete information in respect to your marital status, spouse, number of children or immediate family information (including estimated networth in Ringgit Malaysia (RM)) and all other required information on the first and second pages in the Account Opening Form. 3. Pen down your signature or initial at the bottom of page 2 to page 7 on Alliance Investment Bank Berhad Account Opening form.

Instructions to fill out the forms are located on page 4 of each form.

Supporting documents required -
 a) Two photocopies of the following items b) RM10 for the CDS account opening fee for Bursa Malaysia Securities Berhad.  
 
 
 
NRIC (front and back on a single A4-sized sheet) if you are a Malaysian citizen; or International Passport (all pages on A4-sized sheets) if you are a Malaysian Permanent Resident or a non-Malaysian citizen.

Conventional Bank Statement
Latest active (no later than 6 months) bank statement from a bank incorporated in Malaysia, which the original copy is to be sighted / verified by a Alliance Bank or Alliance Investment Bank officer;
or Online Bank Statement
Latest active (no later than 6 months) bank statement from a bank incorporated in Malaysia months, to be verified by the said respective bank officer of the statement issuer.
